Folliculitis Treatment
Folliculitis is an inflammation of hair follicles that creates a lot of small red or pus-filled bumps that can feel itchy, painful, or burning. It can occur in any part of the body with hair and can include the scalp, face, chest, back, buttocks, thighs, etc. Most cases are caused by an infection (usually with the bacterium Staphylococcus aureus) or other types of infectious agents (like fungi) due to some other reasons as well (e.g., friction, excessive sweating, blockage of hair follicles both internally and externally), but – while folliculitis is usually mild – it tends to become recurrent/less severe due to inadequate treatment.
There are many treatment options available for treating folliculitis, but the first step will be identifying what caused the folliculitis so treatment can be individualized based on severity/prognosis regarding how it will be managed and whether or not to manage symptoms/aggravating conditions, as well as preventing any complications. Generally speaking, the goals for treating folliculitis are to treat any underlying causes of the infection, manage the patient’s symptoms, and reduce recurrences in the future if possible. Uncomplicated cases of folliculitis are often treated with the use of good basic skin care and usually respond to conservative measures. More severe and/or recurrent cases may sometimes require medical therapy.
Patient education in proper hygiene can help to treat uncomplicated odontogenic folliculitis. A person with uncomplicated bacterial folliculitis should avoid wearing clothes that sit closely on the skin; avoid excessive friction on the skin (such as from shaving and irritation), and refrain from scratching the area or the site of the infection, as this can lead to additional dermal complications. Application of warm compresses to the area of infection can relieve discomfort and help facilitate pus drainage from the skin to the warm compress. To improve healing of uncomplicated follicular infections, use of topical antibacterial creams, i.e., mupirocin or fusidic acid, may also be indicated.
For extensive or recurring bacterial folliculitis, oral antibiotics may be indicated based on the suspected or known organism. If there is a concern about methicillin-resistant Staphylococcus aureus (MRSA), then the selection of the antibiotic must be appropriate. In the case of fungal folliculitis (Malassezia), the use of antifungal agents (ketoconazole, itraconazole) is recommended rather than the use of antibiotics.
For folliculitis caused by irritation or ingrown hairs, it is important to modify grooming behaviour. Patients are advised to refrain from shaving the affected area until it heals, use a clean/sharp razor, shave in the direction of hair growth, and apply soothing/antimicrobial products. Acne-like folliculitis may respond to topical retinoids or benzoyl peroxide.
For patients who have chronic or recurrent folliculitis, the provider may need to assess for other possible contributory factors such as diabetes, immune suppression, or carrying Staphylococcus in the nasal passages.

Although folliculitis is usually a mild, manageable skin problem, appropriate treatment prevents future cases, scarring, and additional infections. By determining the cause of folliculitis and utilizing the appropriate targeted therapy along with proper skin care, significantly improved skin health can result from effective treatments of most cases. Most patients will experience complete recovery and have healthy, irritation-free skin with consistent treatment and preventative measures.
